1. Get Employer’s Request for Child Labor form from employer filled out or get a blank one from the Guidance office. If you get your form from the Guidance office take it back to the employer to fill out

 2. Take this completed form back to Guidance.

 3.  Once the Guidance office has the Employer’s Request for Child Labor form they will issue the NH Youth Employment Certificate

 *Please note: Youth Employment Certificates will NOT be issued to a student until the Request for Child Labor form is filled out by the employer.
